  • Information provided to us through software applications we host. As used herein, “our software applications” refers to all of the above.

Our Website and software applications are not intended for children under eighteen (18) years of age. Medical Records and Data You Provide to Us: As you navigate through and interact with our Website, you may be requested to provide your medical records, including but not limited to:

 

  • Medical history and physical exams;

  • Dermatology images, reports, and workups;

  • Lab reports, including type of medical tests that may have been conducted on your blood, urine, tissue, or other bodily fluids; 

  • Medical charts that contain your medical history, including any diagnosis, past medical treatment, and ongoing care;

  • Medical procedure reports, including procedures performed, procedure notes, and post-operative history;

  • Pharmacology history, including details of any medication user is currently taking, as well as any past medication, dosage, frequency, duration of use, and reason for use; 

  • Radiological scans, including imaging reports or scans from X-rays, MRIs, CT scans, Nuclear Medicine scans, PET scans, Ultrasounds, Mammograms, Diagnostic Mammography, and other Radiology scans.

 

The records you provide will be used to offer you a second opinion for medical purposes and for research purposes to enhance the Website’s services and improve its AI capabilities. 

 

Information Collected Automatically: As you navigate through and interact with our Website, we may use automatic data collection technologies to collect certain information about your equipment, browsing actions, and patterns, including:

 

  • Usage details, including the total time you spend on our Website, the time you spend on each page, the order in which those pages were visited and the internal links clicked, the general geographic location from which you access our Website, which browser and operating system you are using to visit our Website, and the referring website;

Protecting the privacy of your medical data is of the utmost importance to the Company. We are committed to complying with the Health Insurance Portability and Accountability Act of 1996 (“HIPPA”) to ensure the confidentiality of your identifiable health information or protected health information (collectively, “PHI”).

 

We will only use your PHI for internal research purposes and to improve our services via our AI software.

We will not disclose your PHI to third parties, except as required by law or with your express consent.

 

We maintain physical, administrative, and technical safeguards to protect the security of your PHI and only authorized personnel will have access to your information.  

 

We do not sell or share your PHI to third parties for monetary compensation.

 

8. Data Security.

 

We have implemented measures designed to secure your personal information from accidental loss and from unauthorized access, use, alteration, and disclosure. The safety and security of your information also depends on you. Where we have given you (or where you have chosen) a password for access to certain parts of our Website or our software applications, you are responsible for keeping this password confidential. We ask you not to share your password with anyone.

 

Unfortunately, the transmission of information via the internet is not completely secure. Although we do our best to protect your personal information, we cannot guarantee the security of your personal information transmitted to our Website. Any transmission of personal information is at your own risk. 10. Data Protection Rights Under the California Consumer Privacy Act (CCPA)

 

If you are a California resident, you are entitled to learn what data we collect about you, ask us to delete your data, and not to share it. To exercise your data protection rights, you can ask us: 

 

  • What personal information we have about you. If you make this request, we will return to you:

    • The categories of personal information we have collected about you.

    • The categories of sources from which we collect your personal information.

    • The business or commercial purpose for collecting your personal information.

    • The categories of third parties with whom we share your personal information.

    • The specific pieces of personal information we have collected about you.

    • A list of categories of personal information that we have sold, along with the category of any other company we sold it to. If we have not sold your information, we will inform you of that fact.

    • A list of categories of personal information that we have disclosed for business purposes, along with the category of any other company we shared it with.

 

Please note, you are entitled to ask us to provide this information twice at any point in a rolling twelve (12) month period. When you make this request, the information we provide will be limited to the previous twelve (12) months.

 

  • To delete your personal information. If you make this request, we will delete the personal information we hold about you as of the date of your request from our records and direct any service providers to do the same. In some cases, deletion may be accomplished through de-identification of the information. If you choose to delete your personal information, you may not be able to use certain functions that require your personal information to operate.

 

  • To stop selling your personal information. If you submit a request to stop selling your personal information, we will stop selling it. If you are a California resident, to opt-out of the sale of your personal information, please email such request to support@doublecheckcare.com.

 

Please note, if you ask us to delete or stop selling your data, it may impact your experience with us, and you may not be able to participate in certain programs or membership services which require the usage of your personal information to function. In no circumstances we will discriminate against you for exercising your rights.
